Your First Visit
At your first visit we will ask you to complete a health history questionnaire and give us the necessary information to complete your insurance claim. We will then perform a complete clinical evaluation and take any indicated x-rays. This will allow us to offer you a diagnosis and discuss treatment options. This first appointment usually includes teeth cleaning.
Health Questionnaire
- We ask general information questions such as address, phone, Physician’s name, and Social Security Number.
- We ask questions on current and past general health conditions and specific dental needs.
- We ask the name and amount of any medications you take.
- We specifically ask about any allergies (mainly medications).
